Technical Assistance:
Discussed with the provider: menu components and topical product authorization form. Provider was also asked to keep an eye on wear and tear of mats.
Comments:
An unannounced monitoring inspection was conducted on 07/10/2023 between the hours of approximately 2:50 p.m. and 3:40 p.m. There were 11 children present with the Provider and 2 assistants for a total of 26 points within the supervision guidelines. The children were observed napping and preparing for snack. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation, not all requirements were met when topical skin products were used.
Evidence: A diaper balm and a butt paste belonging to Child #4 were not labeled with the child?s name.

Description:
Based on record review, the provider failed to obtain documentation of a physical examination prior to a child?s attendance or within 30 days after the first day of attendance for 1 out of 3 children.
Evidence: Child #1 (start date: 04/05/2021) did not have documentation of a physical examination on file.
Plan of Correction:
I informed the parents. They said they are going for 1 week vacation and they will get it by 28th July.
