Comments:
A code compliance inspection was initiated and concluded on 1/8/2024. There were 32 children present with 4 staff supervising. Children were observed during teacher led activities and skill assessment. A total of 4 staff records were reviewed. The inspection started at 10:05 am and concluded at 11:00 am.
Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.
Based on record review and interview with staff, the center did not ensure each person in a supervisory position must be certified by a practicing physician or physician assistant to be free from any disability that would prevent them from caring for children under their supervision.
Evidence: Staff #4 confirmed that the following staff did not have proper documentation by a practicing physician or physician assistant: 1. Staff #1 had no documentation 2. Staff #4's most recent documentation was 7/9/2022
Plan of Correction:
Review all staff files & move up any dates for a health check.
