Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview, and a virtual tour of the program.
A monitoring inspection was initiated on 8/17/2021 and concluded on 8/17/2021. The Provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present 16 points), ranging in ages from 18 months to 4 years, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 child records and 2 caregiver records, and 2 household member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions, please contact me at shahana.green@doe.virginia.gov.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver and household member records, each caregiver and household member did not obtain a subsequent tuberculosis screening every two years from the date of the first screening.

Evidence:
1. Caregiver #1's most recent tuberculosis screening was dated 6/20/2019.
2. Household Member #1's most recent tuberculosis screening was dated 7/22/2019.

Plan of Correction: We made appointments for today and will get them this afternoon.
