Comments:
An unannounced renewal inspection was conducted on December 14, 2023 between the hours of approximately 12:30 p.m. and 1:35 p.m. There were 7 children present with the provider and 1 assistant for a total of 18 points. Children were observed napping. 3 children's records, 2 caregiver records and 2 household member records were reviewed. An inspection of the physical plant, fire drill log, menu, and emergency procedures was completed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the family day home failed to obtain all information for 1 out of 3 children's records.
Evidence: Child #1's (start date:06/21/2023) did not contain a name, address and telephone number for 1 of 2 designated persons to contact in case of an emergency.
