Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on February 23, 2021 and concluded on February 24, 2021. The director was contacted by telephone to initiate the inspection. There were 35 children present and 10 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review and staff interviews, the licensee did not ensure to maintain and keep at the center a separate record for each child enrolled which contains all required information.
Evidence: The record or child #1 did not include the employment hone number for both of the parents listed. The record for child #3 did not include the home phone number for one parent listed.
Plan of Correction:
Center Director has instructed parents of both child #1 and Child 32 to provide these phone numbers as availble or to write n/a if not applicable by end of business today. 2/25/21
