A monitoring inspection was initiated on 8/11/22 after receiving a self-report from the facility in regards to compliance with the regulations. An in-person inspection at the facility was conducted on 8/12/22 where staff were interviewed, the facility was inspected and documentation was reviewed. The investigation was concluded on 4/6/22. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
