Based on observation, the licensee failed to ensure that hanging items, including but not limited to window blind and appliance cords shall be out of reach of children. Evidence: 1. Cords were observed hanging down from a television and an electric air freshener, in a room accessible to and used by children, under five years old. 2. Caregiver #1 acknowledged these cords are accessible to children.
