Comments:
An unannounced monitoring visit was conducted today between the hours of 9:25am through 11:20am with the provider on site. There were 7 children (17 points) with the provider and assistant. A sample of 7 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play and coloring a fish. Interactions between the children and caregivers were positive. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:

2. Child #6 (start date 12/18/2017) was missing the results of the health exam.

Based on record review, six of seven children's records reviewed were not complete.
Evidence:
1. Child #2 (start date 2/12/2019) was missing results of the health exam.
2. Child #3 (start date 4/08/2019) and Child #4 (start date 4/08/2019) were missing name and address of previous child cares.
3. Child #5 (start date 2/04/2019) was missing the medical insurance name and policy number and name and address of previous child cares.
4. Child #6 (start date 12/18/2017) was missing the results of the health exam.
5. Child #7 (start date 2/20/2018) was missing two emergency contact addresses.

2. Child #5 (start date 2/04/2019) was missing the name and address of previous child cares.
