Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.
A monitoring inspection was initiated on and concluded on 7/26/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children present, ranging in ages from 9 months to 2 years old, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 caregiver records were reviewed.

Based on review of three children's records, the provider did not have documentation of a physical in each child's file prior to or within 30 days of attendance. Evidence: 1. Child 1 (start date 1/04/2021) did not have documentation of a physical for the child.
Plan of Correction:
I will ask the parent for it today.
