Comments:
A monitoring inspection was initiated and concluded on 12/01/2021. There were 2 children present, both 14 months old, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 children?s records and 1 caregivers/household record were reviewed.

Based on review of two children's records, the provider did not have documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child #1 (start date 1/04/2021) did not have a physical examination on file with the provider.
Plan of Correction:
I will ask the parent for a copy of it again.
