A complaint regarding multiple concerns including lack of supervision and lack of documentation of injuries was received on February 2 and initiated on February 12, 2020. The complaint investigation was concluded on June 25, 2020. During the investigation staff and a parent were interviewed. Children and staff records were reviewed. The information gathered supports the allegation of lack of supervision and therefore the complaint is valid. A single violation in the area of staffing and supervision was identified and appears on the violation notice. The violation was discussed with the program director at the conclusion of investigation.
Based on observations and interviews the center failed to demonstrate that when staff are supervising children, they shall always ensure their care and protection. Evidence: 1 - On February 25th, child 1 sustained bruises to his back that staff could not explain how, when or why the injury occurred. 2 - Staff 1 stated that during supervision with staff 2,3 and 4 she stated "it was explained to [each staff] the importance of supervising the classes at all times and it was our responsibility to keep the children safe." 3 - Staff 2 stated that child 1 was observed repeatedly banging his head and back on the wall. 4 - Staff 5 stated that she "that child 1 hits himself against the wall."
