Comments:
An unannounced monitoring inspection was conducted beginning at 11:45 a.m. and ending at 2:00 p.m. with the provider and assistant. There were seven children in care, ranging in age from seven months through four years old for a total of nineteen points. Children were observed during lunch. Lunch today was meatballs, green beans, whole wheat roll and watermelon and milk to drink. After lunch, children washed hands and prepared for rest time. Children watched an age appropriate television program while preparing for rest, then had nap. Child care areas including supplies and materials were observed.
Four children's records, two staff records and one household member record was reviewed. One pet record was reviewed.

Based on review of documentation, the provider did not obtain documentation of a central registry background check every five years as required. Evidence: Staff #1 most recent central registry check was dated 12/19/2016. A repeat central registry was due on or before 12/19/2021.
Plan of Correction:
The CRS was done in March, but copy is missing and still awaiting for another copy to come from VDSS.
