Comments:
An unannounced on-site monitoring inspection was conducted on 10/6/2022. There were 93 children, ages 5 months--5 years, and 21 staff members and the director present during the inspection. The inspector observed the following: morning arrival, snack, free choice activities, diaper changing, and outside play. A total of 5 children?s records and 5 staff records, were reviewed. The inspector arrived at 9:50 AM and departed at 1 PM.

Based on review of attendance records, the center failed to ensure that each room has the arrival and departure as it occurs.
Evidence: 1. Reviewed nine classroom?s attendance records and two classrooms did not have completed attendance records. The two classroom attendance records did not accurately indicted the number of children in care.

The director will ensure that attendance is taken correctly. All Staff will monitor.

Description:
Based on observation, the center failed to ensure that infants were not using pillows to sleep.
Evidence:
1. In the infant room, the there was an infant sleeping on the floor on a boppy pillow. The requirement is not for children under the age of two to use a pillow.

The pillows were removed during the inspection. The boppy pillows will not be used when sleeping.

Description:
Based on observation in the Spinners room, the center failed to ensure that all hands were washed as required.
Evidence:
1. Staff was observed changing diapers and used hand sanitizer in between diapers. The requirement is for staff to wash with soap and water after each diaper change.
Plan of Correction:
Staff was reminded that hand sanitizer is not acceptable for hand washing. Director will monitor diapering.

Description:
Based on observation, the center failed to clean and sanitize the diaper changing table as required.
Evidence: 1. The Spinners room staff did not use soap and water to clean the diaper changing table
Plan of Correction:
Staff will use soap and water as the first step in cleaning the diaper changing pad.
