Comments:
An unannounced on-site renewal inspection was conducted on 03/13/2024. There were 100 children, ages 1 months-5 years, and 20 staff members present during the inspection. The inspector observed the following: free choice activities in the gym, nap and snack. A total of 7 children?s records and 7 staff records, were reviewed. The inspector discussed the following with staff: sending links from the DOE website. The inspector arrived at 1 PM and departed at 3 PM.

Based on review of staff?s records, the center failed to ensure that every two years from the date of the initial screening or testing, that staff members shall obtain and submit the results of a follow up TB.
Evidence:
Staff # 3 most recent TB screening was 11/22/2021.

Staff will be sent for a repeat TB screening ASAP. All dates will be monitored.

1.Staff # 5 with hire date of 10/23/2023 and the required two references were dated 10/27/2023 and 10/24/2023. References are to be obtained prior to hire.

In the future all staff will have the required references prior to hire.

Description:
3rd REPEAT VIOLATON SYSTEMIC DEFICIENCY
Based on review of the attendance and interviews with staff, the center failed to ensure that for each grouping of children, the center maintained a written record of daily attendance that documented the arrival and departure of each child in care as it occurred.
Evidence:
1. In the creeper?s 1 classroom two children that were present were not marked present on the attendance sheet at 1:15 PM. Staff # 2 verified that child # 8 arrived at 9:30 AM and child# 9 arrived at 9:20 AM. 2. At approximately 1:20, the creeper?s 2 did not have the daily attendance completed for all seven children present in that classroom. One of the children?s presents was marked present in the Creeper?s 1 classroom but was physically present in Creeper?s 2.
Plan of Correction:
Staff will be reminded to take attendance as it occurs and for all children present in the classroom.

Description:
Based on observation, the center failed to ensure that all prepared infant formula shall be refrigerated, dated and labeled with the child?s name.
Evidence
Observed in the Creeper?s 2 room, three bottles that were in the refrigerator that were not dated and labeled with the child?s name.
Plan of Correction:
During the inspection the bottles were labeled. Staff will monitor all refrigerators.
