Comments:
A non-mandated inspection was conducted at the center and at the main office on 5/13/2024 from 10:15 until 12:11 in response to a self-reported incident concerning child supervision.

A video recording was reviewed, staff interviews were held, and reports were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violation (s) were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on information gathered through interviews, a video recording and a review of documents, the facility failed to provide care protection and guidance of a child in care.
Evidence:
On May 03, 2024, at 12:13pm, child #1 (age 3 yrs. 10 months) was left on the playground for approximately 3 minutes, after two staff took 14 children back to the classroom. Child #1 had left the playground and was found by staff #1 and #2 on the porch of the center. The porch is adjacent to the door to the classroom.

Plan of Correction: Once the Camp Joy incident was reported to Clinch Valley Community Action Head Start?s Education Director, the video for May 3rd was reviewed. After reviewing video, the incident of a child being left on the playground at Camp Joy was reported to the Virginia Department of Education?s department of Licensure, Child Protective Services and the Office of Head Start. Statements from involved employees were gathered. After review of information, and discussion with the Management Staff, Human Resources and the Executive Director, a decision of releasing the two involved employees from employment was made. This was the termination of one teacher?s aide and one substitute. The reason for termination was based on the safety of the children, the disregard of Clinch Valley Community Action Policy, the Office of Head Start Standards, and the Virginia Department of Education?s department of Licensure standards, on sight and sound supervision of children. To further understand what elements may have had an impact of this incident, other reviews and interviews were conducted. One review was that of the Center Directors actions of communication with the other employees. The Center Director was found to not clearly communicate with other employees in the center. Based on these findings, the Center Director of the time of this incident was removed from the roll of center director. This person continued in her role as classroom teacher. The next step of correction included the Education Director, Mental Health/ Disabilities Coordinator, and the Teacher?s Coach, being in the Camp Joy center working, with the remaining employees. This time of being in the center was to monitor, educate and guide employees on how to properly provide sight and sound supervision. These ladies also trained on skills of dealing with problem children and how to redirect children. This training and monitoring period lasted from May 7th through June 5th. The actions taken were swift and warranted in the safety of our most valuable resource, the children we serve.

3.Training of all employees on:
a.Sight and Sound supervision of children.
b.Communication of children placement.
c.Communicate of children counts.
d.Keeping a class together while in activities.
e.Dealing with problem children.
