Provided consultation on: 1) standard 22 VAC 40-185-160B: Acceptable forms of documentation of a negative tuberculosis (TB) screening are 1. A clearance statement signed by a physician, the physician's designee or an official of the local health department; 2. The results of a negative tuberculin skin test (TST); or 3. The results of a chest x-ray negative for tuberculosis disease. 2) 22 VAC 40-185-70A: staff records shall contain the name, address and telephone number of a person to be notified in an emergency
Comments:
An announced focused monitoring inspection was conducted from 1:00 - 4:30 pm offsite at the Massad central office. Six staff records were reviewed. An exit interview was conducted 1/31/2020. Based on review of six staff records, not all staff had a repeat tuberculosis (TB) test/screening at least every two years from the date of the previous test/screening. Evidence: the record for staff #4 had a TB test/screening dated 3/20/17.

Staff will obtain an updated TB test/screening.

Description:
Based on review of six staff records, the facility did not ensure that each staff record contained all required information. Evidence: the records for staff #1, staff #3, staff #4 and staff #6 did not have written information to demonstrate that the individual possesses the education, staff development, certification, and experience required by the job position.

Will ensure that education and experience are documented in the records.
