Comments:
An unannounced monitoring inspection was conducted on 2/7/2023 with center director. The inspection began at approximately 12:30pm and concluded at 2:00pm. There were 54 children present, ranging in ages from 4 years to 5 years old, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, children's records were incomplete. Evidence: 1. Child #1?s record (start date 9/12/2022), did not contain documentation names of persons authorized to pick up the child, and missing the child's doctor's name, address and phone number.
Plan of Correction:
Will reach out to the family of child #1 to receive pediatrician as well as pick up information.

Description:
Based on observation, the center did not ensure that electrical outlets had protective covers. Evidence: 1. During the inspection, the inspector observed five uncovered outlets in the gym. The Pre-K students were observed napping in the gym.

Outlet covers will be covered right away.
