1. Staff #1's (Date of Hire: 8/20/18) record did not contain a tuberculosis test or screening.
2. Staff #3's (Date of Hire: 9/28/17) record did not contain a tuberculosis test or screening.

Before school starts the staff will get TB tests.

Description:
Based on observation of the physical plant, hazardous substances, such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the Upper Elementary classroom, several bottles of cleaning supplies were hanging on the classroom sink outside of the bathrooms. In addition, there were several more bottles of cleaning supplies located in the restroom on shelving. The door to the bathroom was unlocked and accessible by the children in care.
Plan of Correction:
I will speak with the head of the school to determine if we will either lock the bathroom or put the cleaning supplies in a locked box. In the meantime, I will move the cleaning supplies to another classroom where children aren't present until a permanent decision is made.
