Based on record review, the center did not ensure that all children's records contained all required information.
Evidence: Child #1 (Start Date: 9/9/2020) did not have documentation on file of the center verifying the child's identity and age.
Plan of Correction:
Contacted parents again. Was advised they are still awaiting the birth certificate. Parents do not have a birth registry/passport. Parents advised as soon as it arrives in the mail they will bring to school. Parents ordered birth certificate in Aug. 2020.
