Comments:
An unannounced monitoring inspection was conducted on December 12, 2019. Provider was present with four children for a point total of eleven. Four children records and provider record were reviewed.
Based on interview, provider did not ensure there is a written emergency preparedness and response plan that included all required components.
Evidence: Provider could not provide the written emergency preparedness plan during inspection. Provider stated she had printed off the state model form but she had not filled it out yet.
Plan of Correction:
Provider will complete the forms with all required information.
