Violations:
Standard #: 8VAC20-780-245-A
Description: Based on record review, it was determined that staff # 3 did not have documentation of annual training relevant to Health and Safety.
Evidence:
1. Staff had 10 hour Health and Safety Documentation but file did not contain documentation of annual updates.

Plan of Correction: We will request documentation and if not available, we will request the staff complete this years training.

Standard #: 8VAC20-780-500-B
Description: Based on observation of two diaper changes, it was determined the staff did not dispose of diapers in a storage container that is either foot operated or in such a way that a diaper or staff hands do not touch the exterior surface.
Evidence:
1. In Infant's room, staff was observed changing a diaper and staff was seen using hand to lift lid and throw diaper out. Upon closer look, the foot pedal was turned in the other direction so staff could not step on to lift the lid.
2. In the T 3's class, staff member was observed throwing diaper out in a trash can that had a swivel lid so diaper touched the exterior top of trash can.

Plan of Correction: Staff in Infant class corrected direction of step on trash can during the inspection. Staff in other class will be reminded to use the step on trash can located by the sink.
