Comments:
An unannounced monitoring inspection was conducted on 6/24/2024 between the hours of 09:10am-11:10am. 16 children were observed directly supervised by 6 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, and policies were inspected. Children were observed playing outside, looking at books, and playing with blocks. Diapering and handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Based on record review, the center did not meet the proof of child identity and age requirements as stated in the Code of Virginia.
Evidence: 1. Child #1 began attending the center on 5/20/2024. 2. The record for Child #1 did not contain documentation of viewing proof of the child's identity and age.

Evidence: 1. A child was observed sitting in a bumbo seat. The bumbo seat contained safety straps which were not secured.

Based on record review, documentation of emergency evacuation, shelter in place, and lockdown drills did not include all required information.
Evidence: The emergency drill records did not include the following information: 1. Identity of the person conducting the drill; 3. The method used for notification of the drill; 6. Any special conditions simulated; 7. The time it took to complete the drill; 8. Problems encountered, if any; and 9. For emergency evacuation drills only, weather conditions
