Comments:
A Subsidy Health and Safety inspection was conducted on 3/9/20 to review supplemental health and safety requirements. There were 43 children present with 10 staff members. The children were observed engaged in small group activities and playing outdoors. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Based on record review, the center did not obtain for staff documentation of a Tuberculosis (TB) screening within the last 30 calendar days of the date of employment. Evidence: 1. Staff # 1's date of employment was 10/22/19. 2. Staff # 1's TB screening was dated 3/27/19.
Plan of Correction:
Staff # 1 will obtain a current TB screening and for new employees screenings will be obtained within the 30 days prior to employment.

Description:
Based on record review, staff did not complete the Virginia Health and Safety Update training by the date set by the department. Evidence: Documentation of Staff # 1's Virginia Health and Safety Training certificate showed that she completed the training on 3/9/20, and not by 3/1/20 which was the date set by the department.
Plan of Correction:
Trainings will be completed to meet the deadlines.
