Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on November 4, 2019 from 11:15 a.m. through 12:45 p.m. There were thirty-one children in the care of five staff in two classrooms. No medication is being administered at this time. Five children's records were reviewed. Staff records will be reviewed at the Head Start Offices at a scheduled date.

Areas observed were: outside play, transition, hand washing/bathroom procedures, lunch, oral hygiene, and nap. Interactions between the children and staff were observed as appropriate and the children seemed comfortable with their daily routine and schedule.

Standard #: 22VAC40-185-450-A
Description: Based on observation, the center failed to have a bottom sheet for two cots.

Evidence:
1. Nap time was observed in Classroom #1 and Classroom #2.
2. Two cots, in classroom #1, were observed not to have bottom sheets (linens) for the children who were assigned to them.
3. Staff confirmed the children forgot their bottom sheet (linen) and they did not have extra to provide for them.

Plan of Correction: Extra sheets (linens) will be purchased and kept at the center.
