Comments:
An unannounced renewal inspection was conducted on February 28, 2024 from 8:55 a.m. to 10:15 a.m. There were 29 children in care ages three to five years old under the supervision of five staff. Childcare areas, supplies and materials were observed. Children were observed during circle time, during a counting activity and at a variety of centers.
Five children's records, one medication and documentation were reviewed. Emergency drills were reviewed and up to date.
A desk review of staff records was conducted on March 12, 2024. Three staff and four board officer records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of documentation, the center failed to obtain national fingerprint background checks, sworn statement or affirmation and central registry background checks every five years as required. Evidence: Board Officer #3 most recent fingerprint background check was dated 2/5/2019, most recent central registry check was dated 3/19/2019 and sworn statement 1/28/2019. Board Officer #4 most recent fingerprint background check was dated 4/5/2018 and most recent central registry was dated 4/26/2018.

Description:
REPEAT Based on observation, the center failed to ensure that all areas of the center inside and outside were maintained in a clean, safe, and operable condition. Evidence: In the back classroom: 1. There was visible dirt, crumbs and other debris along the edge of the wall area near the tables. Floor tiles had visible dirt and grime. 2. The large blue humidifier had a thick coat of dust along the front vent area. 3. The downstairs bathroom in the back classroom is not operable.
