Comments:
An unannounced inspection was conducted on November 9, 2022 in response to a complaint that was received on 10/11/22 in reference to behavior management, communication with parents, and following the centers own policies and procedures. Interviews were conducted and documentation was reviewed. The preponderance of evidence gathered supports violation of standards.

Violations were cited as a result of this inspection.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today, 11/18/2022. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description: Based on interview and documentation review, the center failed to promptly inform parents when persistent behavioral problems are identified. Evidence: Interview with Director, Curriculum Specialist, Teacher, and Parent, and documentation review, revealed that there were behavioral problems identified with child #1 that caused concern. The child was observed over a four day period prior to the parents being made aware of the behavioral concerns.

Plan of Correction: Per the center "An administrator (and long-time educator) was made aware of the situation by the child's teacher and observed the child to determine if there was a persistent/ongoing behavioral pattern present or if the behavior was developmentally appropriate for a child of that age. Once it became apparent that there was a persistent/ongoing behavioral concern (after four consecutive days of observations), the parents were contacted. We will continue to review our policies and update as needed."
