Comments:
A renewal inspection was conducted on April, 18, 2024 from approximately 10:20 am to 1:35. There were a total of 83 children present, ranging in ages from 2 years to 8 years with 16 staff supervising.
A total of 9 child records and 5 staff records were reviewed. Background checks for the 4 applicants/agents were reviewed.
The inspector reviewed compliance in the areas of administration, staff qualifications and training, physical plant, staffing and supervision, programs, special care and emergencies, and special services.
During the inspection, children were observed participating in outdoor play that included utilizing playground equipment and participating in teacher-directed and self-directed activities. Staff were observed having positive interactions with the children. The center is equipped with toys and supplies and items were available to the children.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it within 5 business days. Please specify how the deficient practice will be or has been corrected.
Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on the review of a child's record (Child #4), and an interview with a staff member (staff #5), the agency failed to maintain documentation of health information as required by 8VAC20-780-140. Evidence: The first day of attendance for Child #4 was 9/14/23. The physical examination in the record for Child #4 was dated 2/12/24. The physical examination was not completed within 30 days after the first day of attendance. In an exit interview on 4/18/24, staff #5 acknowledged that the physical examination in the record was not completed within the required timeframe.

We have contacted the family and will provide additional training to staff.

Description:
Based on observations and an interview with a Center Representative (CR #1), the center failed to provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.
Evidence: The fire inspection report posted at the center was dated 2/22/23. In an interview on 4/18/24, CR #1 reported that the fire inspection report was out of date.
Plan of Correction:
We have established a tracking calendar and training for staff related to maintenance scheduling.
