A Subsidy Health and Safety inspection was conducted on May 1, 2024 to review supplemental health and safety requirements. 6 children were present, ranging in ages from 18 months to 2 years with 2 staff supervising. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it within 5 business days. Please specify how the deficient practice will be or has been corrected.
Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

(Repeat Violation) Based on a review of one staff record (S3) and an interview with a center representative (CR1), the center failed to ensure that all staff who work directly with children shall have current certification in cardiopulmonary resuscitation (CPR) within 90 days of the date of employment.
Evidence: There was no documentation of current certification in CPR in the record of S3. In an exit interview on 5/01/24, CR1 acknowledged that there was no documentation of current certification in CPR for S3.
