Technical Assistance:
A record shall be kept that includes the child's name, date of use, frequency of application and any adverse reactions for diaper ointment.
Sunscreen does not need to be kept locked but shall be inaccessible to children under five years of age or those children in a therapeutic child day program or special needs child day program. Children nine years of age and older may administer their own sunscreen if supervised.
Insect repellent does not need to be kept locked but shall be inaccessible to children. A record shall be kept that includes the child's name, date of use, frequency of application and any adverse reactions.

Comments:
A renewal inspection was initiated on 04/29/22 and concluded on 05/05/22. There were 30 children present, ranging in ages from two to five, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-770-40-D-1-a
Description: Based on record review and interview, the center failed to obtain the findings of central registry background checks for board officers before the 30th day after becoming a board officer.

Evidence:

1. Board Officer 1 does not have a completed central registry background check on file. The director stated board officer 1 started on the board as an officer 8/21.
2. Board Officer 2 does not have a completed central registry background check on file. The director stated board officer 1 started on the board as an officer 10/21.

Plan of Correction: The central registry background check for both staff had been mailed in a timely manner but was never returned. Both staff have completed the paperwork to mail. Once the results are received they will be forwarded to the licensing inspector. In the future the director will ensure central registry background check results not received within 30 days will be follow-up on.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure hazardous substances such as cleaning materials were be kept in a locked place using a safe locking method that prevents access by children.

Evidence:

1. In the bathroom used by children in the Marygold room, there was an spray bottle of bleach solution on an open unlocked shelf.
2. In the bathroom used by children in the Rosebud room, there was two spray bottles of cleaner and Lemon Oil floor cleaner on an open unlocked shelf.

Plan of Correction: The cleaners were moved to a locked location. Staff have been retrained to keep cleaning materials in a locked place.

Description: Based on observation, the center failed to keep hazardous substances not kept in original containers labeled to indicate their content.

Evidence:

1. In the Rosebud classroom two spray bottles of liquid were found on an open shelf in the bathroom used by children. The spray bottles were not labeled.
2. The director was unaware of what the contents of the bottles were.

Plan of Correction: The contents of the bottles will be disposed of or the bottles will be labeled. All staff will be retrained to ensure hazardous substances that are not in original containers are always labeled.
