Comments:
An unannounced focused monitoring inspection was conducted today from 10:00am-11:15am. There were 9 children (27 points) directly supervised by 2 staff. The home, 1 staff record, 2 children?s records, 1 household member?s record, and policies were inspected. Children were observed playing freely, building with blocks, playing numbers bingo, and doing an art project. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-70-A
Description: Based on record review, parents were not provided in writing all required information prior to the child's first day of attendance.

Evidence: Child 1 is missing liability insurance disclosure notice. (Child 1, Start date: 8/22/19)

Plan of Correction: I will give the parent the information today.

Standard #: 22VAC40-111-80-A
Description: Based on observation, proof of age and identity was not obtained within seven business days of the child's first day of attendance.

Evidence: Child 1 does not have documentation that their proof of age and identity was obtained. (Child 1, Start date: 8/22/19)

Plan of Correction: I told the parents already that I need the information.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas outside the home were not kept in a safe and clean condition.

Evidence: In the outdoor play area, there are floor tiles leaned up against the house and several broken and cut tiles stored against the fence accessible to children which causes a potential safety hazard.

Plan of Correction: I will remove the tiles and place them in a safe place away from children.
