Comments:
An unannounced monitoring inspection was conducted on 11/16/23 from 10:30am to 12:30pm with the Director. There were 45 children in care, ranging in age from three-months to five-years-old, supervised by nine staff. The children were observed having free play, transitioning inside from the playground and washing hands, eating lunch, and playing on the playground. Five child records and four staff records were reviewed. The Center has 17 staff with current certification in CPR and First Aid, and 19 staff trained in Daily Health Observation Training. Five medications and authorization forms were reviewed and the center has four staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 3/29/23 and the most recent Health Inspection on file was dated 7/19/23. Lunch served was homemade macaroni and cheese, carrots, applesauce, and milk. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 11/27/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.
Based on review of four staff records, the center did not obtain all of the documentation required for staff records. Evidence: The record for Staff #2 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment.

References were later found in another employee's file, and were completed prior to the staff members start date.
