Comments:
A monitoring inspection was conducted on 9/23/19 from 1:20 pm until 3:30 pm to review licensing standards. At the time of the inspection five children were present with the vendor and an assistant. The sample size consisted of five children's records, the vendor's record, the assistant's record and the members of the household's record. Children and staff were observed during free play, hand washing, preparing snack, snack, clean-up, nap, diapering, outdoor play transitions and behavioral management. Violations were found during this inspection and are documented on the violation notice. If you have questions contact the licensing inspector at (540) 848-4123.
Based on observation and interview, the provider failed to provide families with a menu for the current week's meals and snacks.
Evidence:
1. The menu for today's lunch was posted. 2. The provider stated she did not have the current weeks menu for parents.
Plan of Correction:
A weekly menu will be posted for parents from now on.

Description:
Based on record review, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. The records of five children were reviewed. Child 3's record did not contain documentation of a physical. Child 3's start date was 6/10/19. Child 4's record did not contain documentation of a physical. Child 4's start date was 2/19/19. Child 5's record did not contain documentation of a physical. Child 5's start date was 3/25/19.
Plan of Correction:
The parents of the three children will be advised to provide documentation of physical examinations before the end of the month or they will not be allowed to return until received. In the future parents that do not provide documentation of physicals within 30 days after employment they will not be allowed to return until received.
