Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 8/12/20 and concluded on 8/19/20. The provider was contacted by telephone to initiate the inspection. There were 12 children present with 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 6 children?s records and 1 staff's and 1 adult household member's records submitted by the facility to ensure documentation was complete. Violations from the previous inspection were reviewed to ensure current compliance. Repeat violations were found. Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review and interview, the provider failed to obtain a physical examination prior to attendance or within 30 days after attendance for six children's records reviewed.
Evidence:
1. The provider forwarded documentation of three children's records. Two of the records did not contain physical examinations. The provider stated she did not have any other information for the two children. 2. Child 1's first date of attendance was 7/8/19. Child 3's first date of attendance was 8/14/19. 3. The three missing physicals from the previous inspection were requested. The provider did not have a physical for child 4.
Plan of Correction:
The parents of the children with missing physicals have been told to provide documentation of physicals or they will not be allowed to return. In the future no child will be allowed to attend the childcare until documentation of a physical in received.
