Technical Assistance:
Discussed with the director: infant cribs, cleaning supplies, diapering requirements, nap requirements, medication requirements, resilient surfacing

Comments:
An unannounced monitoring inspection was conducted beginning at 1:15 pm. with the director. There were 69 children in care ranging in age from 3 months old to 5 years old under the care and supervision of 8 staff. Infants were observed having play time on the floor, being fed and playing in infant equipment. Toddlers, two year olds, three year olds and preschoolers were observed during rest time. Classrooms, supplies and materials were observed. The playground was observed. Documentation was reviewed including six children's records and five staff records. Four medications and authorizations were reviewed.

Information gathered during the inspection determined non-compliances with applicable standards or laws and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A-8
Description: Based on review of documentation, the center failed to obtain a written care plan for each child with a diagnosed food allergy from the child's physician. Evidence: Child #4 had a documented food allergy to eggs and milk and did not have an allergy plan in the record.

Standard #: 8VAC20-780-270-A
Description: Based on observation, not all areas were maintained in a safe condition. Evidence: On the two year old playground, there was an exposed screw on the brown barrier that had nine screw lengths exposed that posed an impalement and scratching hazard and rough edges that created a scratching hazard.

Plan of Correction: The director removed the screw during the inspection.

Description: Based on observation, not all potentially hazardous substances were kept in a locked place. Evidence: In the School Age Class, in an unlocked cabinet accessible to children, there was a spray bottle labeled "Mr. Clean".

Description: Based on observation, not all diapering areas were equipped with required items including a leakproof covered receptacle for soiled linens and a disposable diaper disposable system in which the soiled diaper touches the exterior surface of the storage system during disposal. Evidence: In the toddler classroom, the diaper disposal system was a trashcan in which the soiled diaper touches the lid during disposal. In the infant room, the receptacle for soiled linens was a laundry basket with cutouts and was not leakproof.

Description: Based on observation, the center failed to follow the center procedures for administering medication including duration of parent's authorization for nonprescription medication of no more than 10 business days. Evidence: Child #4 had Benadryl on site and the parent authorization was dated 8/2/2021 to 2/2/2022. The 10 day authorization expired 8/13/2021. A second authorization was signed by the parent dated 2/10/2022-8/10/2022. The second authorization expired on 2/23/2022.
