Comments:
An unannounced monitoring inspection was conducted today. There were 12 children present (34 points) with two caregivers at LI arrival. 3 caregiver and 3 child records were reviewed. See Violation Notice for non-compliances cited, Emergency drills, emergency supplies, first aid supplies, cot spacing fridge/freezer temperatures were all in compliance. Based on observation, children in high chairs did not have the safety waist and crotch straps fastened. Evidence: 6 children seated in high chairs did not have the safety strap for the high chair fastened,

Staff was immediately reminded to fasten the safety straps.

Description:
Based on a review of documentation, the provider did not ensure that a caregiver did not exceed 16 points when determining the need for an additional caregiver. Evidence: At LI arrival there were 12 children present totaling 34 points with only 2 caregivers.
Plan of Correction:
A third caregiver arrived within 15 minutes. I am in the process of hiring an additional assistant.
