Technical Assistance:
Discussion was held regarding diapering procedures, supervision, immunizations and hand washing procedures.
Comments:
An unannounced monitoring visit was conducted today between the hours of approximately 9:00 am through 12:00 pm. There were 11 children (26 points) directly supervised by the provider and an assistant. A sample of children's files and staff/household member's files were reviewed. The children and caregivers were observed during the following: free toy play, exercising, lunch, reading stories with the provider, bathroom breaks, hand washing, and other activities. Cribs, the physical space of the care area, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. There were no medications on site today. Areas of non-compliance are identified in the violation notice. Based on observation and interview, the first aid kit did not contain all required supplies. Evidence: 1) The first aid kit contained only one triangular bandage. The first aid kit should contain at least two triangular bandages. 2) The provider stated that the first aid kit contained only one triangular bandage.

I will get a new one.

Description:
Based on record review, the provider did not review annually with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct. Evidence: 1) The most recent review in the record for Child 2 was dated 02/09/2019.

Description:
Based on record review, parent's were not provided in writing of all required information before the child's first day of attendance. Evidence: 1) The records for Child 1 and Child 5 did not contain documentation of having received the Provisions of the family day home's emergency plan.

Description:
Based on observation, caregivers did not ensure that children's hands were washed with liquid soap and warm running water after diapering. Evidence: 1) Staff 4 was observed changing the diapers of 3 children. The hands of the children were not washed with liquid soap and running water after diapering and before the children returning to the group activities.
Plan of Correction:
We will wash the children's hands after changing their diapers.

Description:
Based on observation the diapering surface was not nonabsorbent and washable. Evidence: 1) The diapering surface had small rips on it which prevents it from being nonabsorbent.
