Technical Assistance:
Consultation was given on completing the injury form with a "real time" rather than pick-up for accuracy.
Comments:
An unannounced renewal study was conducted today. Fifteen children were present with two staff and within ratios for children ages 3-5. Observed transition from lunch (bought from home) to hand washing and using the bathroom, reading and puzzles and preparation to go outside to the large center playground. Children wear yellow shirts to help identify them while outside as a large number of community kids also use the playground. Reviewed training and background checks for staff on 2/7/2020 off site at a central location. Areas of non-compliance are identified in the violation notice. Based on record review, 4 of 4 children's records did not contain all required information. Evidence: Child #1 was missing an annual update (signed 8/24/2018) and documentation that proof of identity was viewed. Child #2 was missing a second emergency contact name, address and phone number Child #3 was missing a second emergency contact address and annual update (signed 11/2018) Child #4 was missing was missing an annual update (signed 9/03/2018) and documentation that proof of identity was viewed.
