Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 5/14/20 and concluded on 5/15/20. The provider was contacted by telephone to initiate the inspection. There were 5 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review, the provider did not document the method of verification of a child's age and identity in 2 of 2 children's records.
Evidence: The records for Child #1 (start date 2/26/20) and Child #2 (start date 6/25/18) did not contain documentation of date and person verifying original proof of identification.
