Technical Assistance:
Discussed procedures for administering medication that includes long-term prescription medication with written authorization from the child's physician and parent. A FARE (Food, Allergy & Anaphylaxis Emergency Care Plan) is not sufficient in lieu of authorization forms from the child's parent and physician for long term medications. Comments:
An unannounced monitoring inspection was conducted beginning at 1:45 p.m. and ending at 4:45 p.m. There were 48 children in care ranging in age from 2 months to 5 years old. Infants were observed sleeping and preparing for rest. The toddler class, two year old class, preschool class, prep class and junior kindergarten were all observed during naptime. Classrooms and materials were observed.

Ten children's records and six staff records were reviewed. Seven medications and authorizations were reviewed. Additional information was reviewed regarding a self-reported incident regarding food allergies and staff interviews were conducted.

Standard #: 22VAC40-185-450-A
Description: Based on observation, not all cots were equipped with a top cover and bottom cover during designated rest periods. Evidence: In the Prep classroom, one child was observed during nap without a top cover. In the Junior Kindergarten classroom, two children were observed without bottom covers on their cots during nap.

Plan of Correction: All cots have a top and bottom cover. Emails will be sent to to all families to bring in top and bottom covers. Corrected during the inspection

Standard #: 22VAC40-185-500-B
Description: Based on observation, not all diapering areas were equipped with nonabsorbent surfaces for diapering. Evidence: In the Infant classroom, the diaper pad had an approximately one inch tear in the corner exposing the absorbent material inside and five cracks in the top and bottom of the nonabsorbent surfacing making the diaper pad absorbent.
In the Toddler classroom, the diaper pad had a tear in the corner, exposing the absorbent material inside and three cracks in the nonabsorbent surfacing making the diaper pad absorbent.

Plan of Correction: Diaper pads were ordered for both the infant and toddler classroom on 3/10/2020
