Comments:
An unannounced monitoring inspection was conducted with the director on December 6, 2023, beginning at 1:37pm and ending at 3:36pm. There were 60 children, ages 4 months through 5 years old, in the care of 12 direct care staff. Five children?s records were reviewed. Four staff records were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. Two medications were reviewed and in compliance. The menu, daily attendance, and emergency drill log were reviewed. The most recent practice emergency escape drill was documented as 11/20/2023. The most recent practice shelter-in-place drill was documented as 9/6/2023. The most recent lockdown drill was documented as 2/20/2023. The most recent health inspection was dated 9/13/2023. The most recent fire inspection was dated 6/8/2023.

Standard #: 8VAC20-780-60-A
Description: Based on a review of five children?s records, the center did not ensure all children?s records contained the required information. Evidence: Child A?s record did not have a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: The center will request a written allergy care plan for the child and the documentation will be placed in the child?s record.

Standard #: 8VAC20-780-510-P
Description: Based on review of medication, it was determined that not all medication was picked up within 14 days when the authorization for medication expires. Evidence: The medication authorization form for child A expired April 2022.

Plan of Correction: The center staff has made several attempts to obtain the missing documentation from the parent. The center staff will request and updated medication authorization from the parent.
