Comments:
A renewal inspection was initiated and concluded on April 26, 2022. The licensing inspector was on site from approximately 10:30am until 12:00pm. The provider assisted the licensing inspector during the inspection. There were four children present, ranging in ages from 20 months to three years with one caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. A total of two children?s records and one care provider?s record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-850-90-C
Description: Based on review of two children?s records and interview, the provider did not ensure the required information was in one child?s record.

Evidence:
1. The record for Child #2 (first day of attending 07/19/2021) did not contain the parent's signed authorization to use a substitute provider and his or her name, address, and phone number and any written agreement made between the family day provider and the natural parent, guardian, or other responsible person for each child in care. The agreement may cover hours of care per day, week, or month; cost of care per day, week, or month; frequency and amount of payment per day, week, or month; and any special services to be provided by either party to the agreement.
2. The provider confirmed this information was not located in the record during the time of the inspection.

Plan of Correction: I will have the parents complete the required information and add it to the child's record.

Standard #: 8VAC20-850-110-C
Description: Based on review of two children?s records, observations and interview, the provider did not meet all the criteria on the health and safety checklist at the time of the inspection.
Evidence:
1. Based on observation and interview, the vendor did not maintain all equipment of the family day home in a clean, safe, and operable condition.

Evidence:

1. The provider stated the diaper changing table is used for changing the children?s diapers. The base of the diaper changing table was observed to be broken and sagging underneath the diaper pad, creating a falling hazard.
2. The record for Child #2 (first day of attending 07/19/2021) did not contain a copy of the physical examination results for each child before or within 30 days after enrollment; documentation of adequate immunization received on the date of admission or prior to admission; a copy of the Information to Parents statement and documentation that proof of identity was reviewed and the date of review.

Plan of Correction: 1. I will replace the diaper changing table by this weekend. Most of the children are in pull-ups and are changed while standing.
2. I will obtain the required paperwork from the families and add it the the children's records.
