Comments:
An unannounced monitoring inspection was initiated and concluded on Wednesday, November 16, 2022. The licensing inspector was on site from approximately 9:50am until 10:45am. There was one child present with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and programming. A total of one child's records and one caregiver's records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a review of records and interview, the provider did not meet all the criteria on the health and safety checklist at the time of the inspection.
Evidence: 1) The record of Child #1, date of attendance 10/05/22, did not contain the following: Name, address and telephone number of each parent's place of employment and his or her work hours; name, address and telephone number of one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care; name, address and telephone number of the child's physician; a copy of the physical examination results before or within 30 days after enrollment; proof of adequate immunization received on the date of admission; or documentation that proof of identity was viewed and the date of review.
2) During interview, Caregiver #1 acknowledged the information was missing from the child's record.
Plan of Correction:
The missing information will be requested when the child is picked up. The forms will be given to the family and missing documentation obtained in the next two weeks.
