Comments:
An unannounced complaint inspection was initiated with a local agency on 4/2/2019. A complaint was received by the department on 3/28/2019 regarding the care and protection of children. Licensing Inspector conducted observations, interviewed caregivers, and reviewed records. Based on observations and interviews, there was not a preponderance of evidence to support the claim; therefore, the complaint is deemed Not Valid. Additional violations were found and are documented on the Violation Notice for further details. If you have any questions, please contact me at Ivaco.Clarke@dss.virginia.gov or 571835-5505. Thank you for your cooperation.
Based on interview, the provider did not maintain a record of an injury or accident sustained by a child while at the family day home. Evidence: Caregiver #1 reported that Child #1 sustained a scratch on their upper arm while in care at the Family Day Home on 3/26/2019. The Caregiver also stated that there have been previous incidents where other children have sustained minor injuries while in care. Caregiver #1 further reported that a record of injury reports is not maintained.
Plan of Correction:
I write down everything that happens on injury reports.
