Comments:
An unannounced complaint investigation inspection was initiated on 11/16/2019. The complaint investigation was in response to concerns regarding ratios in the Young Infant and Mobile Infant classrooms. Careful observations, attendance record reviews and staff attendance were made and interviews were conducted. The preponderance of evidence gathered during the investigation does not support the allegations, so the complaint is determined to be not valid. Based on a review of children's records, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: Child #3's record does not have documentation of immunizations by the first date of attendance. First date of attendance is 10/17/19, proof of immunizations dated 11/12/19.
