Technical Assistance:
One pack 'n play onsite. As discussed this piece of equipment may only be used to play and not used for sleeping.
As discussed, infants must be moved every 30 minutes if they are unable to move themselves.
All DOE model forms will be emailed to the provider to use as resources.
Comments:
An unannounced monitoring inspection was conducted today. A total of 3 children were present at the beginning of the inspection and an additional 3 children arrived during the inspection for a total of 6 children (17pts) with the Provider and 1 assistant. The children were observed playing freely in the programming space and interacting with the staff. One child was observed being fed by staff who arrived later. Observation of the programming space to include the second room that had cribs and cots, the bathroom and diaper changing area and the backyard was made. The home was neat and maintained and had toys, equipment and furnishings that were determined age and stage appropriate for the children in care. A total of 4 children's records and the provider's record was reviewed. As a reminder a TB screening is due every two years. The previous violations from the last inspection were reviewed and the corrective actions were verified. As discussed please use the updated DOE forms for enrollment. Based on child record review, the provider failed to review the emergency contact information for one child. Evidence: Child #4's emergency contact information was not reviewed in November 2023.

Based on observation of the family day home, infant bottles were not properly labeled. Evidence: Three infant bottles located in the refrigerator on the lower level were not labeled with a date.

Based on observation of the family day home, one record was not maintained in the home. Evidence: A record of attendance was not maintained.
