Technical Assistance:
Provided consultation on: safe sleep practices and tummy time.

Comments:
An unannounced monitoring inspection was conducted from 11:00 am - 3:00 pm with the center director. There were 63 children, ranging from four months to five years of age, present with ten staff supervising. The children were observed having free play (infants), eating lunch, preparing for nap and napping. Lunch served today: salisbury steak with gravy, whole grain rice, green beans and corn, peaches and milk. Five children's records were reviewed. Four staff records were reviewed and in compliance. Four medications/authorizations forms were reviewed. The emergency drill log was observed and in compliance. Standard #: 22VAC40-185-140-A
Description: Based on a review of five children's records, the center did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: the record for child #1 (date of first attendance: 11/4/19) did not have documentation of a physical examination.

Plan of Correction: Called parent to request missing documentation during the inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation, not all hazardous substances were kept in a locked place. Evidence: there was a spray bottle of cleaning solution and sanitizing solution sitting on an open shelf in the infant room above the changing table.

Plan of Correction: Hazardous substances were moved to a locked cabinet during the inspection.

Standard #: 22VAC40-185-370-1
Description: Based on observation and an interview with staff, not all infants were placed on their back to sleep. Evidence: a four month old infant was observed sleeping on it's stomach; staff demonstrated how the infant was laid down to sleep on it's stomach.

Plan of Correction: Staff will be retrained about safe sleep practices.

Standard #: 22VAC40-185-510-G
Description: Based on review of four medications, the center did not have written authorization to administer medication to a child in care. Evidence: The center did not have a medication authorization form signed by a physician and parent for Child #6 and Child #7's long term medication.

Plan of Correction: Will ask parents to provide medication authorization forms and ensure that they are signed by parent and physician.
