The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 05/15/2019 regarding allegations of administration. The inspection was initiated on November 20, 2019 and completed on 12/02/2019. On 11/20/2019, the inspector was on site from 12:50 pm to 3:51 pm. The licensing inspector conducted an Interview with staff and requested to review available documentation. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.
Based on interview with staff, the center did not ensure to obtain all information for each enrolled child.
