Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the home.
A monitoring inspection was initiated on 08/06/2021 and concluded on 08/10/21. The provider was contacted by telephone and a virtual inspection was conducted. There were 0 children present with 1 caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming and medication. A total of 1 caregiver/household member and 2 children's records were reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, the vendor did not ensure to meet the criteria on the health and safety checklist at the time of the monitoring visit. Evidence: 1. The record of child #2 (start date: 2/9/15) did not have documentation of the the Information to Parents statement and Liability disclosure.
Plan of Correction:
I will get them copied and get the forms signed by parents Tuesday.
