Comments:
An unannounced inspection was initiated and concluded on October 27, 2023 to determine compliance with the requirements for Voluntary Registration. The licensing inspector was on site from approximately 1:00pm until 2:30pm. There were zero children in care and one provider present. The licensing inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication.

Information gathered during the inspection determines non-compliance with applicable standards or law. Violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from the date of receipt. You should specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the stand(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22.1-289.036-A
Description: Based on record review and interview, the provider did not ensure one provider undergo required background checks and every five years thereafter.
Evidence:
1. The record for Provider #1 did not contain documentation of fingerprint-based national criminal background check and results of the Virginia Child Protective Services Central Registry check. Provider #1 confirmed that the required background checks were not in the record.

Standard #: 8VAC20-850-110-C
Description: Based on record review and observation, the provider did not meet the criteria on the health and safety checklist at the time of the monitoring visit.

1. The provider did not take steps to safeguard the home from open and obvious household hazards.
a. The toilet in the downstairs bathroom was removed and sitting in the kitchen. There was an opening in the floor of the bathroom with a towel covering it. The toilet was sitting in the doorway of the kitchen creating a tripping hazard. The toilet was not secured and unstable.
b. The wooden ramp leading up to the front door had several splintered, cracked, and deteriorating wood slats.
c. There were several large areas of chipped and peeling paint and exposed drywall on the wall of the living room and dining room.
2. The provider failed to keep toxic household products in areas inaccessible to children.
a. The entry way contained several large bottles of chemicals on the floor. The bottles of chemicals were accessible to children. The provider stated the chemicals were out because there was water in the basement and there was no other place to store them.
