Comments:
An unannounced monitoring inspection was conducted today between the hours of 12:20pm-3:20pm. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed during storytime, playing outside, coloring, playing with blocks and puzzles, painting, and having snack. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-60-A
Description: Based on record review, 3 of 5 children's records did not contain all required information.

Evidence:
1. The most recent documentation of confirmation of up-to-date information in the child's record for Child #1 (start date 9/2021) was dated 4/12/2021.
2. The most recent documentation of confirmation of up-to-date information in the child's record for Child #2 (start date 10/1/2019) was dated 11/3/2021.
3. The most recent documentation of confirmation of up-to-date information in the child's record for Child #3 (start date 7/20/2020) was dated 11/2/2021.

Plan of Correction: The center is scheduled to send a form to parents of all children for updates this month.

Standard #: 8VAC20-780-550-E
Description: Based on record review, shelter in place procedures were not practiced a minimum of twice per year.

Evidence:
1. A shelter in place drill was practiced on 1/11/2022. There was no documentation of a second shelter in place drill for 2022.

Plan of Correction: The center will add a shelter in place drill to the calendar.
